This form can be used to report a pothole on a County maintained road. Remember, if the road is located within City Limits, report the pothole to that city. If the pothole is on a state maintained road, report it to CalTrans (ie. Hwy 99, Hwy 70, Hwy 162 etc.)

All fields are required.
