Register a Birth
Hospital Births: All births that occur in Butte County hospitals are registered by Public Health Vital Records Office within 10 days.
Unattended Home Births: (unattended by a medical professional) can be registered by the parents within the first year of birth. Complete the California Department of Public Health Out of Hospital application package and contact the Vital Records Office for an appointment.
Attended Home Births: attended by a state licensed midwife must be registered within the first year of birth. Midwives will complete the Midwife Only Worksheet and fax it to the Pubic Health Vital Records Office within three days of the birth. Public Health staff will contact the parents to set up an appointment to register the child. Birth Certificates can be purchased at the end of the appointment. Obtain a Certified Copy of a Birth Certificate
Birth certificates can be ordered in person at the Public Health Vital Records Office. You will be asked to complete the Birth Certificate Application and pay the fee. The certified birth certificate will be printed while you wait.
Certified birth certificates can also be ordered by mail. Complete the Birth Certificate Application, have the applicant signature notarized by a Notary Public, include a check or money order for the fee and mail it to Butte County Public Health Vital Records Office: 202 Mira Loma Drive, Oroville CA 95965
Birth Certificate Amendments
Incorrect information that appears on a birth certificate can be amended, including: spelling errors, parent's data, and hospital errors. Refer to the California Department of Public Health pamphlet Affidavit To Amend A Birth Record for information about birth certificate amendments. A court order will be required to change the child's name. Refer to the California Department of Public Health pamphlet Amending a Birth Certificate After A Court Order Name Change for complete information. A court order will also be required to change the father who is listed on a birth certificate. Refer to the California Department of Public Health pamphlet Adjudication of Facts Of Parentage for information.

Paternity Changes on Birth Certificates
Public Health staff will assist you with Paternity Opportunity Program form completion which is required to add the father’s name to a birth record when the parents are unmarried and no father is listed on the birth certificate.
